Position Summary:
The Health Care & Rehabilitation Center Nurse ensures that bedside delivery of care is person-centered, individualized and clinically competent. Responsible to uphold PRSís core values of excellence, innovation, integrity, stewardship, compassion, and collaboration in fulfilling all job duties.

Job Skills:

  • Knowledge of State and Federal regulations.
  • Knowledge of facility policies and procedures
  • Maintain and promote resident rights, privacy, and dignity.
  • Knowledge and application of medication administration techniques.
  • Maintains the Standard of Practice for Medication Administration as set forth by nursing governing bodies.
  • Knowledge and ability to coach/lead front line team.
  • Knowledge and practice of universal precautions, maintains current with infection control
  • Effectively communicate with residents, families, Health Care representatives and staff.
  • Ability to recognize a change in a residents condition, acts appropriately to these changes,
  • Facilitate needed care.
  • Ability to communicate effectively in writing, in patient records.

Specific Responsibilities:

  • Gather data to assist in maintaining a physical, social, and psychological environment,
    which will be conducive to the best interest and welfare of the residents.
  • Set up and pass medications as needed, observing the 5 rights of medication administration.
  • Maintain working knowledge of resident conditions at all times.
  • Review 24 hour reports and other pertinent nursing documentation from previous shifts.
  • Directly supervise the work of Certified Medication Aides and Certified Nursing Aides.
  • Assign duties to CMA/CNAs and follow up on assigned tasks.
  • Observe universal precautions in providing resident care.
  • Maintain accurate and complete records of cursing observations and care.
  • Observe residents for drug effects/interactions and pain.
  • Provide nursing care in accordance with physician orders and appropriate nursing techniques/procedures.
  • Recognize and interpret symptoms/change of condition and report residentís condition accurately.
  • Notify RCM, DNS, PCP and/or Administration of changes in resident condition or other job related issues.
  • Gather data to assist physician in diagnostic and therapeutic measures.
  • Coordinate, prepare and conduct weekend admissions.
  • Complete incident reports and ensure all necessary steps have been completed prior to submitting to the RCM.
  • Clean and maintain the medication room.
  • Respond to emergency calls on campus (see protocol).
  • Double-check and sign month-end MARs/TARs.
  • Lead the team in the event of an emergency.
  • Check the emergency cart and the emergency team assignments daily.
  • Order medication as needed, per pharmacy protocols.
  • Participate in the Best Practices programs, committees as assigned.
  • Other duties as assigned.


  • Current Texas Licensed Vocational Nurse license required.  
  • Two years nursing experience in Long Term Care preferred.  
  • Current Healthcare Provider Level CPR/AED.
