Position Overview: The Registered Nurse (RN), Case Manager is responsible for enhancing the quality of patient management, maximizing satisfaction, and promoting cost-effectiveness. The ideal applicant will be accountable for the full patient-care cycle including: assessing, planning, implementing, monitoring, and evaluating actions required to meet health and human services between medical team, families, patients, and providers.
Coordinate full-spectrum of care that is safe, timely, effective, and client-centralized.
Ability to manage all aspects of case assignments (draft service plans, review case progress, and determine case closure).
Record patient case information, complete all necessary forms, and produce statistical reports.
Provide patient advocacy and assist in helping patients make informed decisions regarding their treatment options and clinical status.
Promote quality and cost-effectiveness interventions with successful outcomes.
Manage the relationship with social services, health and governmental agencies on behalf of our organization.
Primary source of contact for El Camino Hospital referrals and admissions.
Active CA RN Nursing Licensure.
Active BLS/CPR Certification.
2 years Case Management experience.
2 years Skilled Nursing and/or Long-term Care experience.
Proven knowledge of Case Management principles, healthcare management, professional and technical emerging, and reimbursement process.
Case Management Certification highly preferred (CCM or CCP).
Proficient in Microsoft Word, Excel, and Outlook.
Effective communication, problem-solving, and organizational skills.
