Position Overview:
The Registered Nurse (RN), Case Manager is responsible for enhancing the quality of patient management, maximizing satisfaction, and promoting cost-effectiveness. The ideal applicant will be accountable for the full patient-care cycle including: assessing, planning, implementing, monitoring, and evaluating actions required to meet health and human services between medical team, families, patients, and providers.  
 

  • Coordinate full-spectrum of care that is safe, timely, effective, and client-centralized.
  • Ability to manage all aspects of case assignments (draft service plans, review case progress, and determine case closure).
  • Record patient case information, complete all necessary forms, and produce statistical reports.
  • Provide patient advocacy and assist in helping patients make informed decisions regarding their treatment options and clinical status.
  • Promote quality and cost-effectiveness interventions with successful outcomes.   
  • Manage the relationship with social services, health and governmental agencies on behalf of our organization.
  • Primary source of contact for El Camino Hospital referrals and admissions.  
 
Requirements:
  • Active CA RN Nursing Licensure.
  • Active BLS/CPR Certification.  
  • 2 years Case Management experience.
  • 2 years Skilled Nursing and/or Long-term Care experience.
  • Proven knowledge of Case Management principles, healthcare management, professional and technical emerging, and reimbursement process.
  • Case Management Certification highly preferred (CCM or CCP).
  • Proficient in Microsoft Word, Excel, and Outlook.
  • Effective communication, problem-solving, and organizational skills.
